Support for a smooth transition from hospital, rehabilitation, or skilled nursing. Care coordination, medication management, appointments, referrals, and services are paired with weekly or biweekly check-ins to identify concerns early, address gaps, reduce the risk of readmission, and support a return to baseline.

Chronic Illness Navigation & Advocacy
Coordinate care, track follow-ups, organize information, educate caregivers, and help manage the ongoing pieces of your care. While keeping your goals, priorities, and quality of life at the center.

I actively coordinate appointments, referrals, records, providers, and services. I also educate and guide caregivers to help them understand routines, care needs, safety considerations, and the overall care plan.

Support with difficult healthcare decisions so you can understand your options, ask the right questions, and make choices aligned with your goals.

Support palliative care by coordinating services, helping communicate with the care team, and keeping the person’s wishes and priorities at the center of care. I help address concerns, navigate challenges, and make sure the support in place works for both the person and their family.

How It Works
Intake
In-person intake visit and chart review to understand the full picture. From there, a working plan is developed based on your priorities and needs.
Introduction
Complimentary 20-minute call to discuss your situation, goals, and where support could be helpful. Together, we determine what level of advocacy and navigation would best meet your needs.
Advocacy & Navigation
Hands-on support based on your needs. This may include advocating with your care team, coordinating appointments and services, managing referrals, and handling details behind the scenes so you can focus on yourself or your loved one.

Chronic illness looks different for everyone. I support individuals and families navigating conditions such as stroke, heart disease and heart failure, multiple sclerosis, ALS, autoimmune disease, Parkinson’s disease, traumatic brain injuries, and cancer.

You or someone you love is unexpectedly hospitalized. The first 24 hours can be overwhelming with new information, unfamiliar providers, tests, medications, and decisions happening quickly.
I provide a First 24 Hours Checklist to help you know what to ask and what to address. Bedside advocacy can help organize information, communicate questions and concerns with the care team, and make sure your priorities are heard. When difficult decisions arise, I can help you understand the information being presented, identify questions to ask, and consider how the options align with your goals and wishes.

I help you organize your concerns, understand the perspectives involved, communicate effectively with the care team, and advocate for your rights, needs, and the care you deserve.
Independent of the hospital and insurance company, I work on your behalf, with your needs, goals, and wishes at the center.

The days and weeks after hospitalization can be a vulnerable time. Medication confusion, missed follow-up, gaps in services, and problems that aren't recognized early can contribute to an avoidable return to the hospital.
